Adding Comment Box to a Matrix Item

Is there a way to add a comment box to each individual row within a matrix. For example, we want respondents to rate individual departments on different factors based on a scale of always to never. So each row would be a different department. But, we want to provide a comment box at the end of each row so they can comment on that specific department.

Yes, you can do this easily by adding another column to the matrix and setting it to "Open Ended Single Line". You can put it to the right of the rating scale so that one box will appear for each row in your matrix. I've attached the item xml of a very simple example, which you can import
into your installation if you'd like.
